To get started at DP&IC, simply schedule a "New Patient" appointment today by calling Chiropractor Smith at (815)584-2225. Please indicate you would like a "New Patient Appointment" and provide us with any insurance information you'd like us to check for you. Also, we always recommend you contact your insurance company for a quote of benefits first-hand because insurance policy benefits and contracts are agreements made between the insurance company and the patient, and differ patient to patient. We are in-network with Blue Cross Blue Shield and Medicare and will bill the insurance for you per our agreement(s).
We have early morning and evening hours to schedule early and late appointments for before and after work & school. Our student athletes are provided with doctor notes for any school or practice tardiness that may occur.
DP&IC currently accepts: Health Insurance, Medicare, Workers Compensation Insurance, and Auto Accident Medpay Insurance. Medical referrals for Physical Therapy, Auto and Work Injury care, and After-Surgery therapy are also accepted.
DP&IC will work closely with your: Medical Doctor, Auto Injury Claims Rep, Worker Injury Claims Rep, Employer and Attorney to get you the care you need and deserve.
